Home and Away actress Jessica Falkholt had been clinging to life three weeks after being critically injured in a car smash which killed her parents and sister
The smash in New South Wales on Boxing Day, also killed her parents Lars and Vivian, sister Annabelle and the other driver.
The actress, who played Hope Morrison in the popular Aussie soap, died in St George Hospital on Wednesday morning.
The Home and Away star and her sister were alive when they were pulled from the flaming wreckage near Ulladulla on Boxing Day - but their parents died at the scene.
Jessica's life support was turned off a day after a funeral was held for Annabelle, 21, and parents Lars, 69, and Vivian, 60.
Heartbroken family members told Daily Mail Australia they were at peace knowing the actress is now "back with her family".
They added: "We're all sad but she is with her family now. Her mum was her biggest supporter so I am sure she is in her arms.
"It's an odd feeling, we always knew it was going to happen but no matter what it's a shock."
Jessica's Home and Away colleagues have since paid tribute to the "gorgeous" and "wonderful" actress.A statement from the hospital read: "On behalf of the family of Jessica Falkholt, St George Hospital has been asked to advise the media and the community of Jessica's passing today at 10.20am.
"Jessica passed away from serious injuries sustained in a motor vehicle accident on 26 December, 2017. The family has asked for privacy during this very difficult time."
On Thursday local media revealed the driver of the other car involved who also died in the crash, had previously had a nine-year driving ban and at least 10 convictions for driving while banned.
The man driving the 4X4 which crashed into Jessica's car, serial traffic offender Craig Anthony Whitall, 50, also died.
NSW Police said officers from the crash investigation unit were continuing investigations into the incident following Jessica’s death.
Officers could not confirm if Whitall, who also died at the scene, had been using his mobile phone at the time of the crash.The remains of those killed in the crash were so badly burned, they had to be identified by their dental records, SBS reported.
Witnesses said they saw Whitall's white Toyota Prado cross the central line of the road twice before it smashed into the car carrying the family.
